Everyone is an individual with their own hopes, dreams, goals and needs. We offer a range of services to support you.

Assessment

We want to get to know you. We will talk about what you do in your daily life and how your mental health is impacting on this. Assessment may include informal discussion, observation of functional tasks and formal assessment(s). With your consent we may talk to an important person in your life to gain their insights into the challenges you may have.

Assessments may include:

  • Activities of daily living (ADL)
  • Sensory Profile
  • Cognitive assessment screens to identify deficits that may impact on function, such as attention, memory, problem solving, planning and organisation.
  • Model of Human Occupation Assessments – to assist in identifying your life roles, values, habits, and routines, as well as how your environment may be having an impact on your health and well-being.
  • Social skills assessment
  • Mental health assessment
  • Interest checklists
  • Functional assessments and reports for NDIS

Development of Recovery Goals

We place you front and centre of your recovery journey, and work with you to set goals that you would like to achieve. This may mean breaking a larger long-term goal(s) into smaller achievable steps, so it is less overwhelming.

Once the assessment process has been completed, we will support you to identify and prioritise your recovery goals. These goals are reviewed on a regular basis to identify progress, the next steps in the process and ensure they remain relevant.
